Create Change Wellness Center is a community-based mental health organization, dedicated to providing mental health services to youth and families in North Jersey. Our services include Intensive In-Community (IIC) and Behavioral Assistance (BA), Parent Coaching and Tutoring services.
We are currently looking for experienced licensed mental health providers (LAC, LPC, LSW, LCSW, LMFT) who are devoted to helping youth and families reach their full potential and become their best selves, to join our team.
Clinicians will:
Provide regularly scheduled services to youth and families in the family’s home and/or in their community
Develop and implement individualized treatment plans to meet youth and family’s clinical needs
Become an active participant in the youth and family’s Child and Family Team
Document youth’s progress via electronic record system.
